Please find attached Circular on the above subject for information of all:

Sub : For health checkup of all permanent employees

This is for information of all employees that Health Checkup Programme of our permanent employees has been organized at our Plant Dispensary and will be conducted by M/s. ADD ANNEX HEALTH CARE w.e.f. 8.8.2017 from 8.30 a.m. to 12.00 p.m. and 1.30 p.m. to 5.00 p.m. The check up shall be conducted in two sessions @ 30 employees per session i.e. morning session 30 nos. and afternoon session 30 nos. Employees are requested to carry the IFFCO ID card and to report there in empty stomach. The day wise schedule is attached below. Anybody who fails to undergo checkup in the schedule date due to some unavoidable circumstances can go for mutual change with the colleagues with prior intimation to the undersigned. All sec-heads are requested to inform and release the employees as per the schedule accordingly.
