COVID 19 online Self Declaration Form for employees*

An online system for COVID 19 self declaration has been developed by I.T. Services Department. The system can be accessed form MIS > Web Applications > COVID 19 Compliance System. Employees can access the self declaration form by clicking on Employee's Self Declarations under forms and thereafter clicking on add attendance. Employees can also query their self declaration under the query screen.

The self declaration form has to be filled daily by all the employees without any failure.

Google chrome may be used to open the system and HRMS user id & password to login. In case of any issues I.T. Services Department may be contacted.
