For health checkup of employees with spouse more than45years

This is for the information for all employees that health checkup programme of our employees along with spouse more than 45 years has been organized at ADD ANNEX HEALTH CARE Cuttack. Wef 23/05/2016. IFFCO bus service will be provided for this purpose and 20 persons have to travel daily for health checkup. Bus will start sharp at 7 AM everyday except Sundays and holidays from the township hospital. It is must to carry your IFFCO ID card and to report there in empty stomach. Breakfast and lunch will be provided by ADD ANNEX HEALTH CARE. The day wise schedule will be provided in the circular. Anybody who fails to undergo checkup in the schedule date due to some unavoidable circumstances can go for mutual change with the colleagues with prior intimation to the undersigned. All sec-heads are requested to inform and release the employees as per the schedule.
