please find attached the last and final schedule of Health Check-up at Apollo Hospital, BBSR (up to 16th January 2024), for employees aged 45 and above with spouse. Employees and spouses who failed to attend the Health Check-up on earlier scheduled dates for some reasons are requested to make an entry in the medical department, so that an additional list can be added following 16th January, i.e., before the final closure of the health check-up.
